Homeless Services
As community need has become more pronounced, TMHA has expanded the agency's Homeless Services program, and currently serves over 450 clients annually.
Housing NOW
Housing NOW (formerly 50Now) is a multidisciplinary case management team model developed to house the most vulnerable homeless adults in the County using a Housing First approach. The team is made up of a Nurse, Psychiatric Technician, Therapist and Peer Support Specialist who work to house the most vulnerable homeless adults and has housing choice vouchers through Housing Authority of San Luis Obispo to help that happen.
Permanent Supportive Housing
Permanent Supportive Housing Program has 57 rooms in a variety of houses and apartments for disabled chronically homeless individuals and family members with housing units in San Luis Obispo, Los Osos, Atascadero, Arroyo Grande and Pismo Beach.
Homeless Outreach Team
The Homeless Outreach Team (HOT) is a multidisciplinary team of professionals who work within the community to engage homeless individuals struggling with mental health issues and link them to services and supports to address their mental, physical, and social health concerns.
PATH
PATH (Projects for Assistance in Transition from Homelessness) is a dedicated outreach worker contacting and engaging homeless individuals and family members not currently connected to mainstream services.
Library Outreach Team
Launched in November 2022, the team consists of a part-time Social Worker and full-time Case Manager who connect patrons to much-needed resources, including mental health, housing, food programs, and more.
